Antisera are used in slide agglutination tests. A small colony is emulsified in saline on a glass slide, a drop of antiserum is added, and the slide is rocked gently for up to 2 minutes. Visible clumping (agglutination) indicates a positive reaction — confirming the organism's surface antigen matches the antibody specificity of the antiserum. The procedure requires no instrument and yields results in ≤2 minutes.
Salmonella, Shigella, Vibrio cholerae, and pathogenic E. coli serotypes are all CDC-reportable notifiable pathogens under NNDSS, and serotyping is required for confirmed identification for public health reporting. FDA BAM Chapter 5 also requires serological confirmation with antisera for Salmonella in food safety testing.

Antisera should not be used on mixed cultures — a pure isolate on appropriate selective media is required before serotyping. Rough or autoagglutinating strains may produce false-positive reactions; these must be excluded using a saline control. Antisera serotyping confirms phenotypic antigen expression and does not replace molecular confirmation for definitive speciation in outbreak investigations.

Lateral substitution validation is a standard procedure — typically one shift with ATCC reference strains. Test Principle: Polyclonal rabbit antibodies bind to specific somatic (O), flagellar (H), or capsular (K/Vi) antigens on the surface of bacterial cells. Antigen-antibody cross-linking causes visible agglutination (clumping) within 1–2 minutes on a slide. Absence of agglutination indicates no matching surface antigens.

Limitations: Antisera confirm phenotypic antigen expression only. Autoagglutinating or rough strains will agglutinate in saline control and results are not interpretable. Cross-reactivity between antigenically related serotypes may occur. Do not use on mixed cultures.

QC Requirements: Positive and negative controls must be run each day of use per CLIA/CAP requirements. Document lot number, expiry date, and control results per laboratory's QC policy.
